Synopsis

The purpose of the Fiscal Year (FY) 2027 State and Regional Primary Care Association (PCA) program is to develop and deliver technical assistance (TA) that supports existing and potential Health Center Program award recipients and look-alikes (LALs), hereafter referred to as health centers, both directly and collaboratively with other HRSA-funded TA programs. PCAs will improve the health of medically vulnerable individuals and communities by:

  • Increasing access to high-quality preventive and primary health care services.
  • Improving chronic disease prevention and management to enhance the overall health of communities.
  • Recruiting, retaining, and training a health center workforce.
  • Preparing for, responding to, and recovering from emergent health events.
  • Implementing payer strategies, including value-based care delivery, to ensure high functioning, financially sustainable health centers.
  • Accelerating data-informed improvements to improve operations and clinical quality. 

Who can apply

Applications can be from the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau.New organizations and organizations currently receiving funding as PCAs under Section 330(l) may submit applications. Individuals are not eligible applicants under this NOFO.
