Synopsis

The goal of this cooperative agreement is to improve the overall health and wellness of American Indian and Alaska Native men through a culturally responsive Public Health Nursing (PHN) case management and care navigation model. PHNs will engage men in preventive health and wellness services, increase access to care, promote early identification and management of health conditions, and support healthy lifestyle behaviors through coordinated, community-based interventions.
